Plan Name: Preferred Providers Network (PPO)
Maximum Benefits: up to $750,000 for accident and sickness, as well as Medical evacuation and Repatriation of Remains Benefits, all of which meet or exceed current J-1 visa requirements
Covered Medical Expenses: 100% usual and customary in-network (including Student Health Center) and 80% out-of-network (with US$250 deductible)
Outpatient Prescription Medications, with a copay of $10 for generic drugs and $20 for brand name drugs.
For 2010-2011 policy year, all blood tests related to the physical exam will be covered during an annual physical exam. There is no pre-existing condition.
Dental Plan: Aetna provides 2 options Dental Plan on the voluntarily basis ; Aetna Dental® PPO or Vital SavingsSM* (scholar is responsible for Dental premium payment to AETNA directly).
